Host Lakshman Swamy speaks with Dr. Julius Yang, a hospitalist and Medical Director of Inpatient Quality at BIDMC. Dr. Yang was instrumental in winning a 4.9 million dollar Innovation Grant from the Center for Medicare and Medicaid Services (CMS), to launch a Post-Acute Care Transitions (PACT) program designed to improve patient outcomes and prevent avoidable cost in the high-risk 30-day period following acute care hospitalization. Dr. Yang discusses the complexity in caring for a patient, especially during the transition between the hospital and the primary care practice on discharge. Most importantly, he talks about how systems-level improvements are all about making care better for individual patients.
